J.D., Turk, and Elliot begin their demanding first year as medical interns at Sacred Heart Hospital, juggling chaotic workloads, complicated relationships, and the mentorship and often tough love of senior doctors like Dr. Cox and Dr. Kelso, all while J.D.’s humorous inner monologue frames their growth from uncertain students to more confident young doctors.
When the interns are visited by their parents, they begin to see themselves in a new light. JD realises that his dad's rude manner hides his loneliness, whilst Elliot feels distant from her cold-mannered parents. Meanwhile, Turk is unnerved by the bonding of his girlfriend, Carla, and his brassy mother.
Elliot is attracted to one of her patients, but he doesn't seem to notice. In a last ditch attempt, she chases after him when he gets discharged from the hospital and orders him to ask her out. Elsewhere, Turk and JD disagree over the treatment of one of JD's patients: Mr Hoffner has ulcerative colitis and JD wants him to continue a medical course of treatment with drugs while Turk wants to treat him with surgery.
JD is accidentally exposed to a needle used for testing a patient for hepatitis B While waiting for his test results, JD considers the worst consequences of contracting the disease. Meanwhile, Elliot wonders whether she can handle a new relationship and her heavy workload, and Turk starts on a fitness regime.
JD sees the human side of his mentor when he has to deliver disturbing test results to Dr Cox's good friend and former brother-in-law Ben Sullivan. Meanwhile, Turk almost removes a testicle from the wrong patient and Elliot mistakenly tells her patient Jill that she's pregnant.
Ben's reaction to the news that he has leukaemia is better than expected, but JD finds that his friendship with Ben complicates his professional view of the situation, and Dr Cox reacts fearfully to the news. Meanwhile, Turk's ego takes a knock when he isn't chosen to assist on a difficult surgery, and Elliot is given the opportunity to criticise Dr Kelso's work.
